d. The locks should be standardized by color, shape or size throughout the facility.

A facility may choose to use only red colored locks for the lockout/tagout procedures - this will enable even a casual observer to understand the significance of an installed red lock. Or, a facility may decide to use red colored locks for the maintenance people and green ones for the operators.

e. Each of the installed lock must be accompanied by an informative tag indicating the name of the authorized person who installed it.

This is an important provision. Many times, people forget to remove their locks at the appropriate time, and need to be called back from home to do so. If the authorized person's name is available, it makes it easy to locate him.
